Contact Tab is a free wordpress plugin add a slides out contact form to WordPress site
Main features:
Slides out when tab on left/right of page/post is clicked
Slides in after successful completion of form
Includes three fields- name, email , textbox for message
Captcha to prevent spam (captcha is optional)
Ajax form submit
Appearance of Contact tab such as text color, background color etc can be customized.
Email sent can be forwarded to an additional email address besides the WordPress admin email account.

To install Contact Tab
Login to wordpress (wp-admin).
In your admin dashboard click plugins and select Add New.
Type Contact Tab in the search field and click Search Plugins button.
Install Contact Tab.
After the successfully Installation click the activate Plugin link to activate the plugin.

Unzip and upload it to your WordPress wp-content/plugins directory.
Login to wp-admin. In your admin dashboard click plugins and activate the Contact Tab wordpress plugin.
